Introduction
Progressive Social Service Inc. enables some of our  lower-income clients to obtain home care free of charge through Classic’s Medicaid Service Coordination Unit. Medicaid Service Coordination professionals can assist persons with developmental disabilities in gaining access to  necessary services that meet the needs of the individual on a personalized basis. We are especially proud to help children with Autism connect  with the services they need. These services include:
  • Finding  respite
  • After school programs
  • Weekend programs
  • Transition from school to adult day program

Medicaid Service Coordination professionals can also schedule face-to-face visits to ensure safety and well-being, coordinate services between agencies, help individuals and families assess appropriate and needed services, assist the client in reaching personal goals, and much more.

Case Management

Case managers help provide an array of services to help individuals and families cope with complicated situations in the most effective way possible, thereby achieving a better quality of life. They help families and individuals with deveopmental disabilities to identify their goals, needs, and resources. From that assessment, the case manager works with an individual and/or family to formulate a plan to meet those goals. The case manager helps individuals and families to find resources and facilitates connection with services. Sometimes she or he advocates on behalf of individual to obtain needed services. The case manager also maintains communication with the individual/family to evaluate whether the plan is effective in meeting the individual/family goals.
